Open Question: Help! Where can I find a personal trainer that works around one’s disabilities?

My mother is in a bad way, these days–just over the past few years (she is now in her mid 60’s), she has lost vision in one eye, has become more overweight than ever, has finally lost all cartilage in her knees and needs dual knee replacement as a result, has back problems, very high blood pressure, and a few other challenges she is facing. All this happening to literally the best person I know, and everyone loves her to death.
Suffice it to say, the fact that she is as overweight as she is means that these problems are amplified by quite a lot. We had a personal trainer for her years ago, and she was alright, but she ended up pushing my mother too far, and hurting her further. She was simply a personal trainer one gets at the gym for a few hundred a month, but she did not know how to work with my mother’s disabilities. We don’t know where we could find anyone better or more well suited for getting a disabled woman back in shape.
We live in northern New Jersey. Is there anyone out there that knows where we can look for help? Her health is deteriorating, and I really, really can’t stand by anymore.
Anyone? Please?
